How can I make an appointment?
You can make an appointment through email (info@centenaryphysi.com), calling us at 416-724-4466. Walk-ins are also welcome but please book in advance to make sure we have appointment slots for you.
Are you OHIP funded?
We are not Ontario Health Insurance Plan Funded. But we do offer special pricing for seniors over 65+ if you don’t have extended health care/insurance coverage.
Where is the clinic located?
Centenary Physio & Rehab is located on the main intersection of Neilson and Ellesmere. A Rehabilitation Clinic close to seven oaks. With ample parking space in the plaza.
Do I need to be referred by a doctor?
Both physiotherapists and doctors are able to diagnose your injury. Like doctors, physiotherapists are “first contact” practitioners so they do not need a referral from a doctor in order to examine, diagnose and treat injuries.
What payment options do you offer?
We can direct bill to major insurance companies so you don’t have to pay out of your pocket. If you do not have insurance coverage and are in need of financial assistance please speak to us ; your health is very important to us please speak to us we can help you find funding for your treatments.
Extended Health Care benefits
Centenary Physio and Rehab offers services covered by most health care plans. They typically cover 80-100% fee within the maximum limit. Extended Health Care benefits help plan members and their dependents pay for paramedical services chiropractors, physiotherapists, massage therapist, naturopaths, etc. Additionally, if your insurance company allows direct billing and assignment of benefits we will look after the billing for you. If your insurance company does not allow direct billing, you will have to pay and submit directly to your insurance company and we will assist with your paperwork to make it easier for you. For any treatments that partially covered, you may be asked to co-pay the difference at each visit.
